Accident Summary:
CW2 Salter stated that after a normal pre-flight and a technical inspection by the division artillery maintenance officer of the speed decreaser, he started the aircraft and performed the normal cockpit checks, paying close attention to the antitorque controls. During these checks, all controls were found to operate properly. CW2 Salter then applied pitch and lifted the aircraft to a hover, where after approximately a two second flight, the aircraft experienced a severe shock resulting in complete anti-torque failure. The pilot then executed a hovering autorotation, bringing the aircraft to a safe landing

SUGG^ IN HELICOPTER UH-1C, S/N ^64-14124^. THE AREA UTILIZED FOR VARIOUS STANDARDIZATION MANEUVERS WAS THE "HOVER-TEST AREA", A SOD ARE LOCATED JUST SOUTH OF THE APPROACH END OF ^CAMP HOLLOWAY'S^ RUNWAY 23. AFTER ABOUT AN HOURS FLIGHT, ^CPT WELLER^ DECIDED TO GIVE ^WO SUGG^ SOME STRAIGHT TOUCH DOWN AUTOROTATIONS. THREE OF THE AUTOROTATIONS WERE COMPLETED SUCCESSFULLY BY ^WO SUGG^. THE FOURTH AUTOROTATION WAS ENTERED AT 60 KNOTS ON A HEADING OF 060 DEG. A FLARE WAS EXECUTED AT APPROXIMATELY 100 FEET. Accident Summary:
The aircraft departed Tay Ninh enroute to Nui Ba Den mountain with CW2 Smith at the controls. The aircraft flew around the north side of the mountain and started an approach to the pad located approximately 50 meters from and approximately 50 feet lower than the top of the mountain. Due to the low ceiling, the approach was very flat with almost no angle of descent. At a point approximately 50 meters from the pad, the aircraft was lower then the pad and in a deceleraton attitude. At approximately this point the aircraft started to loose RPM and altitude. At this point it was too late to go around. The aircraft commander attempted to set the aircraft on a boulder and crashed in a 60 debree nose down attitude. The crew and passengers exited the aircraft with minor bruises and scratches, except the aircraft commander who sustained a back injury

Accident Summary:
ON THE MORNING OF 31 MARCH 1971, AIRCRAFT ^65-9440^, A UH-1C, LEFT LZ RATTLER IN QUANG TRI, RVN FOR A TEST FLIGHT, ACCOMPANIED BY ONE OTHER AIRCRAFT. ^CPT BETTENCOURT^, THE AIRCRAFT COMMANDER, WAS SEATED IN THE RIGHT SIDE PILOT'S SEAT. ^PFC HAYES^, THE CREW CHIEF, WAS SEATED IN THE LEFT. THE AIRCRAFT WAS APPROACHING 1200 FEET AGL AT 80 KNOTS, WHEN AFTER APPROXIMATELY TEN (10) MINUTES OF NORMAL FLIGHT, AN EXPLOSION WAS HEARD FROM THE ENGINE COMPARTMENT AREA. ^CPT BETTENCOURT^ REACTED BY IMMEDIATELY ENTERING AUTOROTATION. AT ABOUT THIS TIME, ^MR. COLLINS^, WHO WAS FLYING WING FOR ^440^, RADIOED TO ADVISE THAT THE SHIP WAS ON FIRE. HE REPORTED THAT THE FLAMES WERE BEING EMMITTED FROM THE AFT END OF THE ENGINE. THE FUEL HAD BEEN SHUT OFF IMMEDIATELY UPON ENTERING AUTOROTATION AND WELL PRIOR TO THE REPORT OF FIRE. THE FIRE APPARENTLY EXTINGUISHED ITSELF DURING THE DESCENT AT AN UNDETERMINED TIME AND ALTITUDE. ^CPT BETTENCOURT^ THEN REPORTED THAT HIS N1 AND RPM HAD DROPPED TO ZERO AND THAT HIS EGT HAD RISEN TO 1000 DEGREES. WITH APPROXIMATELY 200 FEET LEFT IN HIS DESCENT, ^CPT BETTENCOURT^ REALIZED THAT HE WOULD NOT GLIDE FAR ENOUGH TO REACH HIS INTENDED LANDING AREA AND WOULD TERMINATE EITHER ON A HILLSIDE OR IN THE BASIN OF A RAVINE. A FLARE WAS INITIATED AND PITCH WAS PULLED AT AN UNDETERMINED ALTITUDE, ALTHOUGH THE AIRCRAFT COMMANDER BELIEVES HE MADE A CONSCIOUS EFFORT AT DELAYING. THE AIRCRAFT LANDED IN A RAVINE BASIN, WHICH AS LINED WITH TALL ELEPHANT GRASS, STRIKING THE MAIN ROTOR BLADES AGAINST THE SLOPE TO THE RIGHT, PRIOR TO THE ACTUAL TOUCHDOWN. THERE WAS NO INJURY TO EITHER CREWMEMBER AND BOTH WERE WEARING ALL THE RET GLOVES AS BOTH MEN HAD PREVIOUSLY LOST THEIR'S. THE AIRCRAFT COMMANDER NEGLECTED TO JETTISON THE AIRCRAFT'S EXTERNAL STORES, PRIOR TO TOUCHDOWN. THERE WERE NO KY-28S ON BOARD BUT ALL RADIOS WERE SALVAGED. THE AIRCRAFT WAS SLUNG BACK TO QUANG TRI BY CH-47.\\

AIRCRAFT ^442^ WAS BEING TEST FLOWN BY ^MR. FINGER^, THE PURPOSE OF THE TEST FLIGHT WAS THE ADJUSTMENT OF THE MAIN ROTOR PITCH CHANGE LINK. HE HAD FLOWN FOR APPROXIMATELY TEN MINUTES AND DETERMINED THE PITCH CHANGE LINK WAS PROPERLY ADJUSTED. HE HOVERED TO THE REFUELING STRIP. THE AIRCRAFT HAD APPROXIMATELY 600 LBS. OF FUEL. THE AIRCRAFT WAS SET DOWN AND THE DOORS WERE OPENED, THE RPM WAS MAINTAINED AT 6600. THE CREW CHIEF BEGAN THE REFUELING PROCEDURE. THE PROPER PROCEDURES WERE FOLLOWED EXCEPT THE AIRCRAFT WASN'T GROUNDED BECAUSE THERE WERE NO GROUNDING WIRES AT THE PUMP, AND THE CREW CHIEF COULD NOT REFUEL WITH HIS VISOR DOWN BECAUSE IT WAS THE TINTED TYPE AND HE COULD NOT SEE BECAUSE IT WAS DARK. THE PILOT WAS GOING TO SIGNAL THE CREW CHIEF THAT THE AIRCRAFT WAS REFUELED BY TURNING HIS NAVIGATION LIGHTS FROM STEDY BRIGHT TO OFF, THEN BACK ON AGAIN. AS THE FUEL GAUGE REACHED 1400 LBS. ^MR. FINGER^ GAVE THE SIGNAL. WITHIN A FEW SECONDS OF THE SIGNAL THE AIRCRAFT WAS ON FIRE. THE FIRE WAS CONCENTRATED AROUND THE FUEL PART AND DOWN THE LEFT SIDE OF THE HELICOPTER, IT SPREAD ONTO THE GROUND AND THE REFUELING HOSE. ON THE MORNING OF 31 MAR 71 AT 0700 HOURS AIRCRAFT ^443^ DEPARTED THE ^116TH ASSAULT HELICOPTER COMPANY^ FOR A DIRECT FLIGHT TO QUANG NGAI. UPON ARRIVAL AT QUANG NGAI ^443^ WAS REFUELED AND DEPARTED TO PROVIDE GUN COVER FOR A COMBAT ASSAULT. AT 1000 HOURS AIRCRAFT ^443^ RETURNED TO CHU LAI TO REFUEL AND REARM. AT 1025 HOURS ^443^ DEPARTED CHU LAI TO PROVIDE GUN SUPPORT FOR ANOTHER CA IN THE AREA OF DUC PHO, REFUELED AT DUC PHO AND COVERED THE CA INTO THE AFTERNOON. AT APPROXIMATELY 1500 HOURS, ^443^ REFUELED AT DUC PHO AND DEPARTED ENROUTE TO THE NEXT LANDING ZONE, AS TRAIL AIRCRAFT, BEING VECTOERED BY THE COMMAND AND CONTROL AIRCRAFT. THE LZ WAS LOCATED IN A SLOPING VALLEY AND WHILE CLIMBING TO THE AREA FOR A RECON, THE ENGINE AND ROTOR RPM BEGAN DECREASING. AT APPROXIMATELY 6200 RPM THE AIRCRAFT COMMANDER TOOK CONTROL OF THE AIRCRAFT AND ATTEMPTED TO REGAIN RPM BY EXECUTING A SLIGHT FLARE. HE THEN ATTEMPTED TO MAKE A POWER ON APPROACH TO THE LZ. Accident Summary:
While flying as wingman in a light fire team escorting a dust-off, the co-pilot observed enemy tracers during a turn. Coming out of the turn the crew chief warned the pilot of trees. Observing the trees, the pilot flared and pulled pitch while continuing into and through the trees. Weather was considered a factor because of light haze, ground fog and darkness.

Accident Summary:
THE UH-1C WAS ON THE FIRST FLIGHT FOR THAT DAY ORIGINATING FROM QUAN LOI. THE FLIGHT WAS ROUTINE AND AT THE TIME OF THE ACCIDENT THE CREW WAS TEST FIRING THE WEAPONS SYSTEM. THEY MADE THEIR FIRST RUN ON THE TEST FIRE ZONE AND EXPERIENCED SOME TROUBLE WITH THE WEAPONS SYSTEM. THEY STARTED A SECOND RUN FROM APPROXIMATELY 300 FEET AGL AND TERMINATED AT APPROXIMATELY 50 FEET AGL WITH A RIGHT BREAK. DURING THE RIGHT BREAK THE AIRCRAFT COMMANDER NOTED A LOSS OF ENGINE AND ROTOR RPM. HE TRIED TO REGAIN THE LOSS BY LOWERING THE COLLECTIVE WITH NEGATIVE RESULTS. THEY CONTINUED TO LOSE RPM AND ALTITUDE. THE AIRCRAFT COMMANDER ENTERED AUTOROTATION AND FLARED TO AVOID A TREE LINE. PICKING OUT THE BEST AREA IN THE IMMEDIATE VICINITY HE ZEROED OUT THE AIRSPEED AND PUT THE AIRCRAFT INTO AN AREA OF 20 TO 30 FOOT HIGH BAMBOO. THROUGHOUT THE ENTIRE FORCED LANDING THE ENGINE REMAINED RUNNING BUT SUPPLYING LITTLE OR NO POWER.\\

Accident Summary:
At approximately 0400 hours, 7 Aug 68 aircraft, 65-9524, a UH-1C, departed LZ Sharon on a mortar patrol mission. After about one hour of flight, it landed at the refueling point at the US Marine facility at Quang Tri Airfield. The pilot reduced power to flight idle and the crew departed the aircraft to refuel. The crew chief noted that there was no fuel pressure in the line and the aircraft commander called the tower to request it be turned on. When the crew chief noticed the line become pressurized, he purged the nozzle, connected the bonding clip to the aircraft and inserted the nozzle into the filler hole on the aircraft. He started to open the nozzle and immediately felt a surge of pressure which forced the nozzle out of the hole, sraying fuel on the outside of the aircraft. Instantly the fuel exploded, blowing him approximately 15 feet from the helicopter. The pilots observed the explosion, secured the aircraft, and departed it uninjured. The crew chief was not injured. The crash crew arrived at the scene in about 2 minutes and extinguished the fire

THE AIRCRAFT HAD REFUELED AT TAY NINH POL AND DEPARTED TO THE SOUTH ON TAKE-OFF HEADING OF 200. AT APPROXIMATELY 100 FEET AGL, 1/4 MILE SOUTH OF TAY NINH BASE CAMP, THE AIRCRAFT HAD A CYCLIC HARD OVER TO THE RIGHT AND WENT INTO A SHARP BANK. THE AIRCRAFT COMMANDER RETURNED TO THE CYCLIC TO THE NEUTRAL POSITION AND BEGAN A DECELERATION TO LAND THE AIRCRAFT IN AN OPEN FIELD. AFTER GETTINT THE CYCLIC TO THE NEUTRAL POSITION, HE THEN HAD A HARD OVER TO THE LEFT AND THE AIRCRAFT COMMANDER WAS ABLE TO COUNTERACT IT BY APPLYING RIGHT CYCLIC AND CONTINUE HIS DECELERATION TO THE GROUND WHEN HE EXPERIENCED A THIRD HARDOVER TO THE RIGHT WHICH CHANGED THE AIRCRAFT HEADING 90 RIGHT. AGAIN THE AIRCRAFT COMMANDER RIGHTED THE CYCLIC AND BEGAN HIS APPROACH INTO A SMALL FIELD. ON SHORT FINAL, THE AIRCRAFT HAD ANOTHER CYCLIC HARDOVER TO THE RIGHT CAUSING THE AIRCRAFT TO STRIKE THE GROUND IN A RIGHT NOSE LOW ATTITUDE. THE AIRCRAFT SLID APPROXIMATELY TEN FEET AND ROLLED TO THE LEFT COMING TO REST UPSIDE DOWN. THE LEFT ROLL OF THE AIRCRAFT WAS STOPPED WHEN THE AIRCRAFT STRUCK A STONE PILLAR.\\

ON 28 JUNE 1969 WO1 STAGG, AIRCRAFT COMMANDER OF A UH-1C WAS IN THE REARM POINT AND HAD BEEN TOLD BY CU CHI TOWER TO HOVER TO ROW BRAVO FOR REPOSITIONING FOR TAKEOFF. PRIOR TO TURNING THE CORNER TO ROW BRAVO THE RPM STARTED TO DECREASE. WO1 STAGGS ATTEMPTED TO GET THE AIRCRAFT TO A GRASSY AREA ON ROW BRAVO. AT THIS TIME THE 20 KNOT TAIL WIND CAUGHT THE TAILBOOM AND SPUN THE AIRCRAFT 90 DEG TO THE RIGHT. WO1 STAGGS APPLIED LEFT PEDAL BUT THE AIRCRAFT FAILED TO RESPOND. THE AIRCRAFT LANDED HARD ON THE LEFT REAR SKID FROM A HEIGHT OF 3 TO 4 FEET. WO STAGGS ALERTED THE CREW BEFORE IMPACT AND THAT THEY SHOULD HOLD ON TIGHT BECAUSE THEY WERE GOING DOWN HARD. Loss to Inventory
I was AC and LFT leader (Diamondhead 50) in this Charlie with Cobra wing. Friendly ground element was pinned down at base of mountain surrounded and taking heavy machine gun fire from enemy positioned up slope at about 200 feet elevation. The LFT expended on various targets, primarily on the slope. Friendlies called for critical medevac, previous attempts were aborted due to
heavy AA fire. We were low on fuel and expended except for door gun, and advised ground we would attempt evac. As aircraft was landed, the tail was turned toward mountain to allow passenger boarding while crew maintained door gun fire. The wounded were 1 walking and 1 stretcher. The aircraft took heavy fire on touchdown, the walking wounded made it on board, the stretcher did
not. The crew chief was wounded and takeoff attempted. Wingman advised aircraft on fire. Aircraft made forced landing after flying about 100 meters and slid into bomb crater. Aircraft had sufficient rotor speed to back out of crater, and crew and passenger exited burning aircraft. The crew then E & E'd approximately 800 meters to armor column. Wing (CWO Watson) made repeated low level passes although expended to cover crew which was still receiving heavy
fire, the enemy fire was directed at his ship as he did this. Door gunner
(Henley) carried wounded crew chief (Hearn) piggy back nearly the entire
distance (occasionally spelled by pilot Schenk) through 4' elephant grass.
After arriving at the armor column entire crew was evac'd to Tay Ninh by

AIRCRAFT WAS ENROUTE TO CU CHI AFTER AN EXTRACTION. DIAMONDHEAD 10, THE FIRE TEAM LEADER, WAS THE LEAD SHIP OF TWO GUNSHIPS AND ONE TROOPS SHIP. AT FIRE SUPPORT BASE HUNTER, THE FLIGHT BEGAN FLYING LOW LEVEL. BETWEEN FIRE SUPPORT BASE PERSHING AND CU CHI BASE CAMP, DIAMONDHEAD 10'S WINGMAN MADE A CALL ASKING THE LEAD AIRCRAFT TO SLOW UP AS HE WAS FLYING AT 90 KNOTS AND COULD NOT MAINTAIN PROPER SEPARATION. THE LEAD AIRCRAFT THEN WENT INTO A CLIMB TO AN ALTITUDE OF APPROXIMATELY 500 FEET. IT THEN NOSED OVER INTO AN EXTREMELY STEEP DIVE TOWARDS THE GROUND. THE WINGMAN STATES THAT THE AIRCRAFT NEVER PULLED OUT OF THE DIVE, BUT ANOTHER WITNESS STATES AT THE BOTTOM IT APPEARED TO LEVEL OFF. SKID MARKS PRESENT WHERE INITIAL CONTACT WAS MADE INDICATE THAT THE AIRCRAFT STRUCK THE GROUND WITH WHAT APPEARED TO BE A TREMENDOUS FORWARD GROUND SPEED. THE AIRCRAFT CONTINUED TO CARRY ALONG THE GROUND ALMOST 200 METERS FROM THE POINT OF INITIAL CONTACT. THE AIRCRAFT DISINTEGRATED AS IT CONTINUED IN ITS FORWARD PATH ALONG THE AXIS OF TRAVEL.\\

ON THE MORNING OF 12 JULY 1970 AT 0630 HOURS, AIRCRAFT ^024^ DEPARTED THE ^176TH ASSAULT HELICOPTER COMPANY^ FOR A DIRECT FLIGHT TO TIEN PHUOC AIRFIELD WHERE THEY REFUELED AND SHUT DOWN. AT 0800 HOURS, ^024^ DEPARTED TIEN PHUOC ON A COMBAT ASSAULT ESCORT MISSION IN THE VICINITY OF KHAM DUC. THEY FLEW 01:30 AND THEN RETURNED TO TIEN PHUOC FOR REFUELING AND REARMING. AT 1000 HOURS THEY DEPARTED ON A CH-54 ESCORT MISSION, AND RETURNED TO TIEN PHUOC AT 1130 HOURS. THEIR NEXT MISSION OF THE DAY WAS ANOTHER CH-54 ESCORT MISSION WHICH BEGAN AT APPROXIMATELY 1500 HOURS. AS ^024^ PREPARED TO TAKE-OFF ON THIS MISSION, ^WO1 FATHEREE^ DISCOVERED THAT DUE TO THE INCREASING DENSITY ALTITUDE, HE HAD DIFFICULTY IN MAINTAINING A STABILIZED HOVER. Accident Summary:
AT APPROXIMATELY 1115 HOURS, ON 16 AUGUST 1969, LT VARGAS AND HIS GUNSHIP CREW WERE SCRAMBLED FROM THE TAY NINH VIP PAD TO SUPPORT TROOPS IN CONTACT. AT THE TIME OF THE SCRAMBLE TAY NINH WAS EXPERIENCING LIGHT RAIN SHOWERS WHICH HAMPERED VISIBILTY. LT VARGAS' AIRCRAFT WAS PARKED IN AN ADJOINING AREA TO THE VIP PAD, BETWEEN TWO REVETMENTS, AND SITTING ON A HEADING OF AOBUT 200 DEGREES. ADJACENT TO THE VIP PAD IS THE WEST ROAD WHICH IS THE NORMAL DEPARTRUE PATH FROM THE VIP PAD ON A HEADING OF 200 DEGREES. THE AIRCRAFT WAS SITTING 25 METERS TO THE RIGHT OF THE ROAD, MEANING THAT ON DEPARTURE LT VARGAS WOULD HAVE TO FLY TO HIS LEFT UNTIL REACHING THE WEST ROAD AND CONTINUING HIS CIMBOUT DOWN THE ROAD. AFTER STARTING THE AIRCRAFT AND CALLING FOR TOWER CLEARNACE, LT VARGAS VISUALLY CHECKED HIS NORMAL TAKE-OFF PATH ALONG THE WEST ROAD AND NOTED A CONVOY PARKED ON THE FAR SIDE OF THE WEST ROAD. HE ALSO NOTED A UTILITY POLE ABOUT 50 METERS TO HIS LEFT FRONT AND 25 FEET HIGH. RECEIVING CLEARNACE FOR DEPARTURE, HE PERFORMED A HOVER CHECK AND INITIATED A MINIMUM POWER TAKE-OFF OVER THE ROAD. UPON ATTAINING TRANSLATIONAL LIFT AT APPROXIMATELY 20 KTS OF AIRSPEED, LT VARGAS TURNED SLIGHTLY TO THE RIGHT TO AVOID OVER FLYING THE CONVOY. LT VARGAS FAILED TO REALIZE HIS PROXIMITY TO THE UTILITY POLE AND STRUCK IT WITH HIS MAIN ROTOR BLADE. THE AIRCRAFT NOSED TO THE RIGHT IMPACTING IN A DRAINAGE DITCH. THE AIRCRAFT THEN ROLLED TO THE LEFT AND CAME TO REST ON ITS LEFT SIDE. ALL THE OCCUPANTS EXITED THE AIRCRAFT WITHOUT SERIOUS INJURY.\\

Accident Summary:
The flight of three helicopters had been engaged on a sniffer mission north of Nui Ba Den and were returning to Cu Chi after refueling at Tay Ninh. The flight was composed of one UH-1D flying in the lead at 2500 feet approximately 1/2 mile in front of the gunships. The UH-1C and AH-1G were at 2000 feet with the UH-1C leading in a tactical trail. The time was 1500 hours on 4 October 68. At 1505, the AH-1G aircraft commander notified the UH-1C aircraft commander that he was coming up on his right wing to fly formation. The aircraft commander of the UH-1C "rogered" this transmission and continued on his flight path. After approximately one minute of formation flight, the Cobra aircraft commander noted he was too close to the UH-1C. This fact was also noted by the doorgunner of the UH-1C who mentioned it to WO Robert Moore. The aircraft commander of the AH-1G then applied right cyclic to move his aircraft away from the UH-1C. At that time, the rotor blades of the two aircraft made contact. The AH-1G immediately entered an unusual attitude and then further went into a steep descending left turn. Both pilots of the Cobra state the aircraft was almost uncontrollable and was vibrating severely. The aircraft commander made an emergency transmission over the radio. Both pilots were attempting to control the cyclic and the aircraft commander was controlling the collective pitch. The pilot was unable to assist the aircraft commander with the antitorque pedals due to the extreme vibration. As the aircraft neared the ground both pilots executed a flare to dissipate their airspeed. All available collective pitch was applied and the aircraft struck the ground in a relatively level, but slightly sideways attitude. The skids of the Cobra were spread evenly. The aircraft rolled over on its right side. The pilot exited through his door using the emergency jettison handle. The aircraft commander exited through the left side of his canopy using his breakout knife. Both pilots were not wearing their clear visors down because they interfered with their vision. The pilot was wearing gloves but the aircraft commander was not. Their sleeves were rolled down and helmet chin straps were fastened. In the meantime the UH-1C was descending straight ahead experiencing a severe 1 to 1 vertical vibration. The aircraft commander had also made an emergency transmission. It required the efforts of both pilots to maintain control of the aircraft. No turns were made in the descent and the aircraft was landed with power in approximately two feet of water. After landing the aircraft was shut down as expeditiously as possible, and the crew set up a small defensive perimeter. The crew of the UH-1D, after hearing the emergency transmission executed an immediate left turn and saw both aircraft going down

War Story:
UH-1C 66-15168 was hovering into the gunship rearming area. There was a tailwind - Pilot sitting on left side sated he had run out of aft cyclic. Aircraft got extremely close to another UH-1C that was at flight idle also starting to rearm. AC bottomed pitch to stop aircraft and the main rotor blades of the aircraft (66-15168) struck the main rotor blades of the UH-1C 66-15210. The AC of 66-15210 had unbuckled and was departing his aircraft to supervise rearming as the blades struck causing 66-15210 to lurch up tail coming right and to roll over on its top right side killing the AC with a fatal head injury. The main transmission mast and rotor system of the hovering aircraft (66-15168) came out and flew approximately ten feet before coming to rest nearly intact. There were no injuries on 66-15168. Both aircraft sustained major damage to main rotor blades, rotor heads, mast, transmission mounts and transmissions. There was no fire. from Army Aircraft Accident Report Check List

PRIOR TO THE MISHAP THE AIRCRAFT WAS INVOLVED IN A TOUCHDOWN AUTOROTATION, WHICH IS BELIEVED TO HAVE CAUSED THE MAIN ROTOR BLADE TO FLEX DOWN & PARTIALLY SEVER THE TAIL ROTOR DRIVE SHAFT. THIS WAS UNKKNOWN TO THE IP & ON THE NEXT TAKEOFF FROM DONG TAM AIRFIELD, DIRECTIONAL CONTROL WAS LOST & THE AIRCRAFT SPUN 360 DEGREES TO THE RIGHT. THE AIRCRAFT WAS AT 30 FEET & 40 KNOTS AT THE TIME THE MISHAP OCCURRED. THE PILOT REDUCED THROTTLE & LANDED WITH NO FURTHER DAMAGE TO THE AIRCRAFT. MAINTENANCE PERSONNEL REPLACED THE NUMBER FOUR SECTION OF THE TAIL ROTOR DRIVE SHAFT & RELEASED THE AIRCRAFT FOR FLIGHT BACK TO UINH LONG AIRFIELD.\\

Accident Summary:
The flight of three helicopters had been engaged on a sniffer mission north of Nui Ba Den and were returning to Cu Chi after refueling at Tay Ninh. The flight was composed of one UH-1D flying in the lead at 2500 feet approximately 1/2 mile in front of the gunships. The UH-1C and AH-1G were at 2000 feet with the UH-1C leading in a tactical trail. The time was 1500 hours on 4 October 68. At 1505, the AH-1G aircraft commander notified the UH-1C aircraft commander that he was coming up on his right wing to fly formation. The aircraft commander of the UH-1C "rogered" this transmission and continued on his flight path. After approximately one minute of formation flight, the Cobra aircraft commander noted he was too close to the UH-1C. This fact was also noted by the doorgunner of the UH-1C who mentioned it to WO Robert Moore. The aircraft commander of the AH-1G then applied right cyclic to move his aircraft away from the UH-1C. At that time, the rotor blades of the two aircraft made contact. The AH-1G immediately entered an unusual attitude and then further went into a steep descending left turn. Both pilots of the Cobra state the aircraft was almost uncontrollable and was vibrating severely. The aircraft commander made an emergency transmission over the radio. Both pilots were attempting to control the cyclic and the aircraft commander was controlling the collective pitch. The pilot was unable to assist the aircraft commander with the antitorque pedals due to the extreme vibration. As the aircraft neared the ground both pilots executed a flare to dissipate their airspeed. All available collective pitch was applied and the aircraft struck the ground in a relatively level, but slightly sideways attitude. The skids of the Cobra were spread evenly. The aircraft rolled over on its right side. The pilot exited through his door using the emergency jettison handle. The aircraft commander exited through the left side of his canopy using his breakout knife. Both pilots were not wearing their clear visors down because they interfered with their vision. The pilot was wearing gloves but the aircraft commander was not. Their sleeves were rolled down and helmet chin straps were fastened. In the meantime the UH-1C was descending straight ahead experiencing a severe 1 to 1 vertical vibration. The aircraft commander had also made an emergency transmission. It required the efforts of both pilots to maintain control of the aircraft. No turns were made in the descent and the aircraft was landed with power in approximately two feet of water. After landing the aircraft was shut down as expeditiously as possible, and the crew set up a small defensive perimeter. The crew of the UH-1D, after hearing the emergency transmission executed an immediate left turn and saw both aircraft going down

Accident Summary:
AT APPROXIMATELY 1030 HRS, ^8 OCT 69^, THE SPECIAL FORCES CAMP LOCATED AT ^CHI LANG^ CAME UNDER ROCKET AND MORTAR ATTACK. A LIGHT FIRE TEAM WAS SCRAMBLED AS COUNTER-DEFENSE. UPON EXPENDING THEIR ORDNANCE, THEY RETURNED TO ^CHI LANG^ FOR POL AND REARMING. ^CW2 NICHOLS^ REFUELED AND PROCEEDED DIRECT TO THE WESTERN MOST PSP AREA FOR ROCKET REARMING. ^CPT SHULTZ^ PICKED UP FOUR (4) BOXES OF 7.62MM AMMO AT THE OLD REARMING POINT, HOVERED TO POL, REFUELED AND PROCEEDED TO THE WESTERN MOST PSP AREA FOR ROCKET REARMING. UPON REACHING THE REARMING POINT, ^CPT SHULTZ^ HOVERED SLOWLY INTO POSITION TO THE LEFT OF HIS LEAD, WHO WAS ALREADY IN THE PROCESS OF REARMING. ^CW2 NICHOLS^ AND HIS PILOT HAD EXITED THEIR ASSIGNED AIRCRAFT, WHICH WAS STILL RUNNING AT 5500 RPM. AS HE POSITIONED HIMSELF BESIDE THE LEAD AIRCRAFT, HE TURNED HIS NOSE SLIGHTLY TO THE RIGHT, SITTING DOWN DIRECTLY FACING THE ROCKET CONEX. AFTER TOUCHING DOWN, ^CPT SHULTZ^ BOTTOMED THE COLLECTIVE CENTERED THE CONTROLS, ROLLED THE THROTTLE TO FLIGHT IDLE AND TURNED THE FORCE TRIM TO THE "ON" POSITION. AT THIS TIME, A LOUD EXPLOSION WAS HEARD. AS ^CPT SHULTZ^ SET HIS AIRCRAFT DOWN AND BOTTOMED THE COLLECTIVE, THE SKIDS WERE NOT FULLY SPREAD ON THE PSP AND HE HAD SLIGHTLY OVERLAPPED HIS MAIN ROTOR WITH THAT OF AIRCRAFT ^#536^. WHEN THE WEIGHT OF ^CPT SHULTZ'S^ AIRCRAFT FORCED THE SKIDS TO SPREAD ON THE PSP, THE TIP PATH PLANE OF HIS ROTOR LOWERED INTO THE TIP PATH PLANE OF AIRCRAFT ^#536^, CAUSING A BLADE STRIKE. THIS CAUSED THE TRANSMISSION OF ^#533^ TO EXIT TO THE LEFT REAR AND MAST BUMPING. MAST BUMPING BECAME SO SEVERE THAT IT SEVERED THE MAST, SENDING THE ROTOR BLADES TWO HUNDRED (200) FEET TO THE REAR. THE MAIN ROTOR'S REARWARD FLIGHT CUT THROUGH THE VERTICAL STABILIZER, CUTTING THE DRIVE SHAFT CAUSING THE TAIL ROTOR TO SEPARATE. AT THE POINT OF CONTACT THE BLADE OF A/C ^#536^ WAS ON TOP FORCING THAT BLADE TO STRIKE THE GROUND AFTER A 180 DEGREE ROTATION. AS THE BLADES MADE CONTACT,THE AIRCRAFT TRANSMISSION EXITED TO THE RIGHT. UPON BLADE CONTACT WITH THE GROUND, THE AIRCRAFT TILTED TO THE RIGHT. AS THE WEIGHT SETTLED BACK ON THE SKIDS, THE CROSS TUBES GAVE WAY, BRINGING THE AIRCRAFT TO REST ON IT'S UNDERSIDE.\\

Accident Summary:
AIRCRAFT WAS WORKING AN AREA APPROXIMATELY 15 MILES SW OF KONTUM CITY, VIETNAM. THE MISSION AT THE TIME WAS TO PROVIDE COVER FOR B TROOPS' SLICKS ON THE INSERTION OF THEIR AERO-RIFLE PLATOON. CW2 TUSI PICKED UP AND BEGAN TO PARALLEL THE SLICKS AT ALTITUDE OF APPROXIMATELY 300' AGL. AS THE SLICKS TOUCHED DOWN IN THE SELECTED LZ, CW2 TUSI BEGAN A CLIMBING LEFT HAND TURN FROM APPROXIMATELY 100' AGL. DURING THE CLIMBOUT FROM THE LZ, THE COCKPIT FILLLED WITH SMOKE. HE GAVE CONTROL OF THE AIRCRAFT TO HIS COPILOT, WO1 DECKER, AND BEGAN PULLING CIRCUIT BREAKERS IN AN ATTEMPT TO LOCALIZE WHAT HE THOUGHT TO BE AN ELECTRICAL FIRE. HE WAS PULLING CIRCUIT BREAKERS WHEN BOTH HE AND HIS COPILOT HEARD A SINGLE LOUD BANG COME FROM THE ENGINE COMPARTMENT. UPON HEARING THE NOISE, CW2 TUSI TOOK CONTROL OF THE AIRCRAFT AND CHECKED HIS INSTRUMENTS. THE TACHOMETER SHOWED THE NEEDLES TO BE JOINED AND RPM DECAYING RAPIDLY. HIS FIRST ACTION WAS TO REDUCE COLLECTIVE AND INITIATE AN AUTOROTATION. HE ENTERED THE AUTOROTATION AT APPROXIMATELY 200' AGL. THE PILOT SAW THAT HE WASN'T GOING TO MAKE THE AREA HE HAD SELECTED FOR A FORCED LANDING SO HE THEN MADE A SHARP LEFT TURN TO GO TO ANOTHER AREA THAT HE COULD MAKE, CW2 TUSI CONTINUED THE AUTOROTATION TO THE AREA, EVEN THOUGH IT PUT HIM MOSTLY DOWNWIND. THE AUTOROTATION WAS SUCCESSFULLY COMPLETED INTO A RICE PADDY. UPON TOUCHDOWN, THE MAIN ROTOR BLADE SEVERED THE TAIL BOOM APPROXIMATELY 8-12 INCHES FORWARD OF THE 42 DEG GEAR BOX. THERE ARE SIGNS OF YELLOW PAINT FROM THE MAIN ROTOR BLADE ON THE LETTER "G" IN THE WORD "DANGER" (PAINTED ON THE LEFT SIDE OF THE TAIL BOOM). ALSO THERE IS YELLOW PAINT FROM THE MAIN ROTOR BLADE ON THE RIGHT LONGERON. UPON TOUCHDOWN, THE AIRCRAFT DID NOT HAVE ANY GROUND RUN AS IT SETTLED IN 2-3 FEET OF MUD. BOTH AVIATORS EXITED THE AIRCRAFT. CW2 TUSI WAS UNABLE TO SHUT THE AIRCRAFT DOWN SO HE FINALLY REMOVED THE SCREEN FROM THE ENGINE OIL COOLING COMPARTMENT AND SHUT IT DOWN MANUALLY.\\

THIS MAJOR AIRCRAFT ACCIDENT INVOLVED TWO HELICOPTERS, AN AH-1G AND AN UH-1D 66-00897, WHICH COLLIDED IN MID-AIR DURING A COMBAT MISSION IN RVN. THE AH-1G AIRCRAFT WAS OBSERVED TO STRIKE THE UH-1D AIRCRAFT IN ITS RIGHT AFT SECTION WHILE THE UH-1D WAS FLYING LEVEL AT APPROXIMATELY 1200 FEET. THE MAIN ROTOR HEAD OF THE AH-1G SEPARATED AT THE MAST AND THE AIRCRAFT WAS OBSERVED TO FALL TO THE GROUND AND BURST INTO FLAMES. THE TWO CREW MEMBERS ABOARD THE AH-1G AT THE TIME OF THE ACCIDENT SUSTAINED FATAL INJURIES. THE UH-1D AIRCRAFT WAS OBSERVED TO MAKE AN AUTOROTATION WHICH TERMINATED IN A HARD LANDING. War Story:
George Grinnel was the back-seater on this flight and was the AC. He was just finishing up his 2nd or 3rd tour, was engaged to be married, and was due to go home about the time of this incident. I believe he was engaged to a girl he met on R&R in Hong Kong, but it's been over 30 years, and I could be mistaken. I witnessed this mid-air between the Diamond Head cobra and a slick from another (unknown) unit. I was working as the low level (OH-6A) bird on a D Trp. 3/4 Cav. Hunter-Killer team at the time. I don't remember who was my high bird at the time. We were working an area known as the sugar mill on the Siagon River, on the Cambodian border, south west of Cu Chi. We saw the cobra and slick fall, and I headed for the cobra as it was going down. When I saw the cobra explode on impact, I diverted to the slick, which had crashed, but there was no post crash fire. There were survivors of the crash, and I think maybe everybody survived. My gunner, PFC Rocky Rhodes, and my CE, SP4 Steve Snoddy, got out and assisted in the recovery of the wounded. I wrote an after accident report as part of the accident investigation, and I may still have a copy of my hand written report. Hope you can correct the AC entry as George was a real hero in my book. I have pictures of him and his crew when he was flying UH-1C gunships prior to the unit getting AH-1Gs, after they shot up a .51 Cal position, and the gun was captured by the grunts. The .51 Cal ended up out in front of Diamond Head Ops as a display. I was with B Co., 25th Avn, 25th Inf., with George and Bob, until I was transfered to D Trp. 3/4 Cav., after they lost most of their LOHs and needed replacement LOHs and crews. ON 31 JULY 1969, A FIRE TEAM WAS SCRAMBLED FROM B CO., 25TH AVN BN TO ESCORT A DUST OFF SOUTH OF SAIGON XS 7972. ENROUTE TO THE LOCATION THEY CAME UPON HEAVY RAIN AND LOW CEILINGS, 400 TO 600 FEET. WHILE ENTERING THE CU CHI TRAFFIC PATTERN THE WING SHIP IN THE FIRE TEAM ENCOUNTERED HEAVY RAIN AND LOW SCUD CLOUDS, LOST SIGHT OF THE BASE CAMP AND HIS LEAD SHIP, AND ENTERED IFR CONDITIONS. THE AIRCRAFT COMMANDER OF THE WING SHIP, WO1 DENNIS D COLLETT BECAME DISORIENTED AS THE AIRCRAFT BEGAN TO SETTLE WITH POWER, AND WERE UNAWARE OF THE ATTITUDE OF THE AIRCRAFT. AT APPROXIMATELY 100 FEET ALTITUDE THEY SPOTTED TREES COMING UP ON THEM, LEVELED THE AIRCRAFT AND CUSHIONED THE LANDING WITH COLLECTIVE PITCH. THE AIRCRAFT HIT HARD AND ROLLED OVER ON ITS LEFT SIDE. Accident Summary:
AIRCRAFT 403 WAS ON A LOCAL TRAINING FLIGHT AND HAD TAKEN OFF FROM FSB RAWLINGS TO FLY A CLOSED TRAFFIC PATTERN FOR PRACTICE MANEUVERS. THE INSTRUCTOR PILOT WAS GIVING A QUARTERLY STANDARDIZATION CHECK RIDE AND HAD BEEN TRAINING FOR APPROXIMATELY ONE AND ONE HALF HOURS. ALL MANEUVERS AND RESPONSE OF THE AIRCRAFT UP TO THE TIME OF THE ACCIDENT HAD BEEN NORMAL. WHILE ON CROSSWIND LEG OF THE TRAFFIC PATTERN, AT APPROXIMATELY TWO HUNDRED FEET AGL AND SEVENTY KNOTS AIRSPEED, THE NOSE OF THE AIRCRAFT YAWED SHARPLY TO THE RIGHT AND DROPPED. THE AIRCRAFT SPUN 180 DEGREES TO THE RIGHT WITH THE PILOT APPLYING LEFT PEDAL. THE LEFT PEDAL HAD LITTLE, IF ANY EFFECT IN STOPPING OR SLOWING THE SPIN. A LOUD GRINDING NOISE WAS HEARD FROM THE REAR OF THE AIRCRAFT DURING THE HALF TURN SPIN. AFTER THE MEMENTARY STOPPAGE OF THE SPINNING, THE AIRCRAFT CONTINUED SPINNING TO THE RIGHT AT AN INCREASED SPEED. THE INSTRUCTOR PILOT TOOK THE CONTROLS AS THE RATE OF DESCENT INCREASED. DURING THE DESCENT, THE SEPARATED PORTION OF THE TAILBOOM WAS SEEN FALLING TO THE GROUND. THE INSTRUCTOR PILOT REDUCED THROTTLE TO OVERCOME THE SPINNING BUT IT HAD NO EFFECT. AT APPROXIMATELY FIFTEEN FEET THE INSTRUCTOR PILOT APPLIED FULL PITCH TO DECREASE THE RATE OF DESCENT. THE AIRCRAFT HIT THE GROUND UPRIGHT AND REMAINED UPRIGHT. THERE WAS NO GROUND RUN AFTER LANDING. THE PILOT SHUT THE ENGINE DOWN AND EXITED THE AIRCRAFT. THERE WAS NO FIRE OR IMJURIES.\\

AT ABOUT 1700 HOURS, ^LT MACK^ AND ^MR. WHITTLE^ DEPARTED CU CHI ON THE MISSION OF GIVING ^MR WHITTLE^ HIS 90 DAY STANDARDIZATION CHECK RIDE. BOTH THE PILOT AND INSTRUCTOR PILOT PREFLIGHTED THE AIRCRAFT PRIOR TO DEPARTURE AND FOUND NO DISCREPENCIES THAT MIGHT PRECLUDE A SAFE FLIGHT. AFTER LANDING AT THRUNG LAP TO CLEAR THE AREA, THE AIRCRAFT MADEA NORMAL TAKEOFF AND ESTABLISHED A NORMAL TRAFFIC PATTERN FOR AN AUTOROTATION. THE FIRST POWER RECOVERY AUTOROTATION WAS COMPLETED SATISFACTORILY. THE SECOND AUTOROTATION WAS PROCEEDING NORMALLY UNTIL ^MR. WHITTLE^ ATTEMPTED THE POWER RECOVERY TOO LATE. ^LT MACK^ TOLD HIM TO TAKE IT TO THE GROUND. THE AIRCRAFT MADE A FAIRLY SMOOTH LANDING WITH ABOUT FOUR FEET OF GROUND RUN. THE COLLECTIVE CONTROL WAS LOWERED, A NOISE WAS HEARD IN THE VICINITY OF THE TAIL SECTION, AND INVESTIGATION REVEALED THAT THE TAIL BOOM HAD BEEN SEVERED BY THE MAIN ROTOR BLADES.\\
